In written updates the same clarity is missing: the last two project notes buried the decision in paragraph four. Lead with the decision.” The second version names the situation, the behaviour, the effect it had, and one concrete thing to change. That is the standard to aim for, and producing it reliably is what quality nudges in 360 feedback software are trying to achieve.
